Lambert- Eaton myasthenic syndrome (LEMS) is a paraneoplastic disease that is associated with voltage-dependent calcium channel antibodies. These autoantibodies affect especially presynaptic neuromuscular area. A 65-year-old male patient admitted to hospital with the complaint of difficulty in walking and climbing stairs. LEMS was considered with clinical examination and electrophysiological assessment. The diagnosis of lung adenocarcinoma was done by thoracic computed tomography and bronchoscopic biopsy. LEMS is especially associated with small cell lung cancer. It is rarely associated with lung adenocarcinoma like this patient. In the electrophysiological anlyses; significant amplitude increment is diagnostic with high frequency tetanic stimulation.
